How To Register For Course(s)
You can easily enroll with CPT, if you meet the following qualifications:
1. Must have a High School Diploma with a GPA of 2.0 or higher, or have a GED.
2. Must be able to attend our classes in Southern California. We have many class sessions designed for individuals with busy schedules (i.e. weekend classes).
If you meet these qualifications and would like to register, follow the steps below:
Print out the PDF version of our registration packet. Once completed, you can submit the packet by mail or fax.

Note: You are required to complete all three sections (Basic, Advanced, and Clinical).
To be enrolled in desired class, please complete the registration form and mail with the minimum deposit to the address on the form. The packet must be received no later than the Monday prior to the first class.
Class sizes are limited, so register early!
There will be no refunds of any payment made to CPT for classes that the student is unable to attend. Deposits and/or payments may be used towards future classes. A change in decision of class dates will incur a fee of $50.00.
Pre-Registration is required for all of our courses, no "Walk-Ins" will be allowed without prior consent of CPT administration.
With signing of these registration forms you are giving your permission for a background check required by the hospitals contracted with us for externship rotations. This will include research of your criminal history, court reports, and contact with law enforcement agencies. By signing these papers you release and hold their respective officers, directors and employees harmless from any and all liability with respect to the investigation, verification and/or use of any information relevant to your registration.

NEW STATE CERTIFICATION REQUIREMENTS ---CLICK HERE!!!
The state of California, as of April 9, 2003, has implemented certification requirements for those who currently practice phlebotomy, or wish to do so. All phlebotomists and students must take a California approved phlebotomy course, and upon completion, pass a national exam. If you have had phlebotomy experience in the last 5 years, you may only need our advanced class to be eligible for the certification test and licensing required by the state of California.
Note: Students from other schools are welcome to take our Clinical courses, based on certain eligibility requirements.
